Requirement of Applicant 
1. Age 18 or above.  (Age under 18, need to be signed and declared by parent/guardian.
2. Valid freediving certificate, need submit copy for reference. 

Membership yearly fee
HK$250 per annual.
For those who enroll the course, you can waive this application procedure and 1st annuel fee. 

Important Note 
1. Membership valid from 1st Jan to 31st Dec. Membership renewal could be processed from 1st Dec for next year.
2. After submit application form and paid, it means you agree to be bound by the rules, regulations and decisions of HKFDA.
3. It is advisable all students to complete all classes during the membership period. After the membership expired, it will be necessary to pay an additional annual fee for renewal before continuing to attend the remaining classes.
4. HKFDA reserves the right to the final approval of the membership application.
